Instructions: This survey was designed to analyze the state of your organization’s health. Circle the description that best represents your opinion of the following statements as they apply to your organization. There are 63 questions in the OHA. SCORING THE ORGANIZATIONAL HEALTH ANALYZER

Instructions: Circle the response you gave to each item. Sum across circled numbers and add the total for each illness. That with the highest count is the most probable illness. The higher the count, the greater the probable strength. For any single illness, use the following scale:

21 or less = normal; 22 – 28 = possible illness; 29 or greater = probable illness

In addition, add the total count for all illnesses. A total of 180 or more may indicate possible illness even if no illness has other than a normal reading.

What To Do About Conflicting Sets of Symptoms

Many symptoms you observe are common to more than one illness. Usually, this is not a problem because we already corrected the OHA for variances due to this fact. Sometimes, however, more than one illness is present and you won’t get clear-cut results with the OHA. This may also be due to false symptoms and temporary symptoms. On the rare occasion when the OHA does not clearly distinguish among illnesses, you must use the same techniques as that of a psychologist or medical doctor in determining which are real and which can be ignored. This includes using your common sense and trusting your gut feeling.

To decide, look at the severity of the symptoms as well as their number and persistence. Are they always present, or do they appear sporadically? Are they growing or fading? Eventually, you will find that one ailment is far more likely. When that happens, your diagnosis is 90% complete. If sets of symptoms are about equal in strength and do not fade, note down all illnesses that are present. You can all use the Diagnosis/Illness Matrix as a crosscheck.
